Emblem will be using three networks in 2020: Prime is their largest network with coverage in NY, NJ & CT. Select Care will be a smaller network available in NY. Millennium will be the smallest network only available in NY.

1st quarter 2021 monthly rates:
New York City, Westchester and Rockland counties:
Prime Plans - These plans use the larger Tri-State Emblem Prime network. All major hospitals are included throughout the Metropolitan Tri-State area. This plan is comparable to the Oxford Liberty network. Some plans are no referral ( non-gated ) some require a referral ( gated ). POS plans will include national coverage using the FirstHealth national network, No limit on enrolling out-of-state employees into the POS plans.
Select Care Plans - These plans use the smaller Select Care network. These plans are for the Metro NY City area. These plans do not include Sloan Kettering, NYU, the LIHN, Westchester Medical or Maimonides hospitals. Some plans are no referral ( non-gated ) some require a referral ( gated ).
Millenium Plans - These plans use the smallest Emblem network. These plans are for employees in NY City ( all Burroughs )Nassau, Suffolk, Rockland and Westchester counties. These plans use the Catholic Long Island Health Network of Hospitals, Mount Sinai and NY Presbyterian hospital Systems.

EmblemHealth currently only offers HMO plans to small businesses. Most of Emblem's HMO plans require a referral from your primary doctor to see a specialist. Before enrolling in an EmblemHealth HMO, please verify your doctors and hospitals accept the EmblemHealth HIP Prime network in NY. The HIP Prime network is a very large network of doctors and hospitals in the New York City and Long Island regions. EmblemHealth plans now use theTristate network: Emblem Prime in NY, Qualcare for NJ and ConnectiCare for CT. for all of their small business plans.
