These plans use the national Cigna OAP network or the smaller Multiplan PHCS network or the NJ only Qualcare network.    These plans renew 12 months after enrollment.  Enrollment can be the 1st of the month for any month of the year. The deadline for enrollment is the 18th of the month prior to the effective date.   Prices start at $753.50 per month.  These plans are underwritten for initial acceptance.  

2. Patriot National MEC Health Plans: National Cigna and PHCS network plans for individuals, groups or self-employed (View Quotes)  (View Ultra MVP SBC)      (View Ultra MEC SBC)    (View Essential benefit Summary) 
These MEC plans are our lowest cost plans that includes hospital in-patient benefits and surgical benefits.   These are $0 deductible plans with maximum visits per plan year for most services.  Premiums start at $456.50 per month.

3. Patriot Blue Cross PPO Health Plans:  Anthem(National Blue Card network) for Individuals, The Self-employed or groups:(View Quotes)    These plans use the national Anthem Blue Card PPO network.  These plans renew 12 months after enrollment.  Enrollment can be the 1st of the month for any month of the year. The deadline for enrollment is the 18th of the month prior to the effective date.   Prices start at $763. per month.

4. Union membership plans:   As a member you will have access to EmblemHealth Prime HMO and POS Health Plans (Self-Employed)

We are affiliated with a local union that accepts the self-employed as members.  If you join this union, you will have access to EmblemHealth HIP Prime HMO plans and POS plans and other benefits as a union member.  These health plans are for self-employed residents of NY, CT or NJ.  In New York these plans use the EmblemHealth HIP Prime network, this is a great network in NY.   In NJ these plans use the QualCare network, In CT these plans use the Connecticare network. These plans are fully insured, there is no medical review.  These plans are Platinum level plans with $0 deductible.  Open enrollment all year.   Please call or write for information.   NEW!
