1. EmblemHealth EPO and PPO plans Large New York network, no referrals, large national network, EPO plans offer $0 co-pay for children's office visits. HSA plans offer very low cost premiums. These plans give your employees national coverage. These plans require a minimum 50% enrollment (after valid wavers) of your full time employees to qualify. These plans are available in all NY State counties. Uses the Multiplan network nationally.
1a. Emblem Comprehealth HMO Plans are traditional HMO plans with low copays and low monthly premiums. Available for Long Island, New York City and Westchester small businesses only. Very limited network of providers. Will enroll down to one eligible employee if there are 2 to 50 employees eligible.
2. Oxford Health Plans (a Unitedhealthcare company) EPO, POS and HSA plans are very affordable, 2 Great networks to choose from, Freedom and Liberty. These plans give your employees national coverage. These plans can enroll down to one employee from groups of 2 to 50 employees. Available in all downstate NY counties up through Ulster and Sullivan Counties. Uses the UnitedHealthcare network outside of the tri-state area. Oxford sells its plans with or without prescription plans, If you are interested in quotes without prescription plans please contact us for custom quotes. Oxford will now require 51% minimum employee participation after valid waivers( effective 2/15/12 for new business), also Oxford will allow for up to 75% out of area employees, this will be good for groups terminating from Empire with high out of area employee enrollment. Oxford will still allow for executive carve outs, but Oxford has to be the sole insurance company for the business.
2a. Oxford Health Plans Liberty HMO is a very low cost traditional HMO plan with low copays and a very good network of providers. Can enroll down to one employee from groups of 2 to 50 employees. Available in all downstate counties ( Nassau, Suffolk, New York City, Westchester, Orange, Putnam, Rockland and Dutchess Counties). The Liberty HMO still has no participation requirements, This is the best option for groups that will be enrolling less than 50% of full time employees after valid waivers.
3. Empire Blue Cross Blue Shield Plans is scaling down their small business health plans. If your small business currently has Empire Blue Cross, please contact us for an alternative insurance company.
4. HIP Health EPO & PPO Plans includes preventative dental and vision. Good local network, no referrals, large national network. Great family rates. Available in New York city, Long Island, Westchester, Rockland and Orange Counties. These plans require a minimum 50% enrollment (after valid wavers) of your full time employees to qualify ( minimum of 2 employees must enroll ). Uses the PHCS network nationally. Only downstate carrier to still pay at 80 UCR on out of network claims with PPO plans.
4a. HIP Health Plan of NY HMO Plans includes preventative dental and vision. Good local network, with or without referrals. Great family rates. Available in New York city, Long Island, Westchester, Rockland and Orange Counties. No minimum employee participation, will enroll down to one employee from groups of 2 to 50 employees.
4b. HIP and Emblem EPO & PPO quotes through the Long Island Association. These quotes are good for businesses located on Long Island, New York City, Westchester and Rockland counties. The LIA offers your employees a menu of health insurance choices under one group bill.
4c. Oxford, HIP, and Emblem quotes through Health Pass. Like the Long Island Association Health Pass offers your employees a menu of choices under one group bill. These quotes are good for businesses located on Long Island, New York City, Westchester, Rockland, Putnam, Orange, Ulster, Dutchess and Sullivan Counties
5. Aetna EPO, PPO and HSA qualified high deductible plans are available in all counties in New York State. Aetna requires 50% enrollment (after vaild waivers) of your full time employees to enroll to qualify for these plans. Very good local and national network.
5a. Aetna NYC Community plans are very low cost HMO plans for businesses located within New York City only. Limited NYC network, approximately 25% of the NYC Aetna providers accept this plan. These plans will enroll down to one employee from groups of 2 to 50 employees.
6. 7. MVP Health Care has a good network of doctors throughout New York State. MVP is available In most counties North of NY City. MVP EPO, PPO and high deductible Plans require at least 75% of your full time employees to enroll (after valid waivers). HMO plans will enroll down to one employee of groups of 2 to 50, HMO plans have no participation requirements.
8. UnitedHealthcare Is one of the largest health insurance companies in the country. Very good local and national network. Available in all New York counties in upstate NY North and West of Ulster and Sullivan Counties, all other New York counties are serviced by Oxford Health Plans ( a UnitedHealthcare company ).
9. CDPHP: EPO plans are a good value with low rates in Eastern and Central New York State. These plans will require a minimum of 50% enrollment of all full time employees to qualify. PPO plans and high deductible plans are also available, contact me for additional customized quotes. CDPHP has the most choices of deductibles and copays of any NY state health insurance company. Very good no referal coverage with national network access using the First health national network outside of New York.
10. Excellus Blue Cross Blue Shield Has PPO and High deductible plans in three regions in NY state ( Utica region, Syracuse region and the Rochester region) These three regions cover from Delaware county up to Clinton county and as far west as Stueben, Monroe and Livingston Counties. Excellus also offers sole proprietor plans in these areas.
11. Independent Health Buffalo region (Allegany Cattaraugus, Chautauqua, Erie, Genesee, Niagara, Orleans and Wyoming counties).
